I <3 TH Sculpture Request Form

Interested in bringing the "I <3 TH" Sculpture to your Downtown Terre Haute event? Then you've come to the right place! You must fill out this form to indicate your interest. 

Here are the requirements:

• Your event must be located in the designated Downtown Terre Haute corridor.

• Your organization must pay for the DTH Add-On benefit in its Terre Haute Chamber of Commerce membership OR be an event that Downtown Terre Haute is sponsoring in-kind. 

• You are responsible for any damage to the sculpture while in your possession. 

• You accept liability for any personal accidents in or around the sculpture.
